Create questionnaires to gather information and store it in the appropriate place during the intake process on Litify. Make sure to plan your questionnaire before you build it and create questions before building your questionnaire.
Note: In the Questionnaire Output field, all mapped fields on a given node are shown first. This may cause your question answers to appear out of order.
1. In Litify Setup, click the More downarrow and select Questionnaires.
2. On the Questionnaires page you can view, edit, delete, sort, and create questionnaires. Click NEW in the upper-right corner.
3. Complete Questionnaire Builder.
- Name the questionnaire.
- Determine if the questionnaire isActive and Available. See this article for complete information about active and available questionnaire settings.
- Active questionnaires can be set as the Default Questionnaire on intakes.
- Available questionnaires appear in the dropdown list of questionnaires when you add new questionnaires to an intake.
- Click the+button to add question nodes. Nodes contain sets of questions that appear to a user at once within a questionnaire. Nodes allow you to group questions. See this article for complete information about nodes.
- Click the little pencil next to the node to name and describe it for clarity of navigation. See this article for complete information about naming nodes.
- Click the node to add questions.
4. Click the Check CQC? box if you want this node to check every CQC rule you have built. This occurs when users press Next on the questionnaire.
- Set the navigation with the two dropdown lists. If any CQC rule is triggered when a user pressesNext, the questionnaire will navigate from this node to node 1 on the Animal Accident questionnaire. If you want to navigate to the next node on this questionnaire after a CQC rule is triggered, you must select it in the dropdown lists. Note: If you do not fill out the navigation and a CQC rule is triggered, you will not be able to navigate further in a questionnaire.
- Check the Pass navigation back to previous node box to direct navigation to the node previous to the current node. See this article for complete information about this setting.

5. Now that you have added a node, you must add questions to the node. There are four areas to complete: a. Question, b. Description, c. Mapping, and d. Required.
- Click the plus sign to add a question to the node. Select a question from the dropdown list. This list contains every question you have built.Note: Once you add questions, you cannot reorder them in the node. Add them in the correct order at this stage.
- Describe the question in the Description field. A question's description appears beneath the question on the questionnaire. Use this as help text for intake agents or other users completing questionnaires.
- Map the answer to a field to streamline your process. You can map a question's answer to a specific field on the record, so the answer auto-populates the selected field in the Mapping dropdown. For example, if your question is "When did this automobile accident occur?" and the answer type is Date-Time, you can map this question to the "Incident Date" field.
- Determine if the question is required. If you check the Required box, users will have to answer the question to navigate further in the questionnaire.
Next, define the Navigation of the node. Navigation rules are required to navigate to sequential nodes in the same questionnaire or nodes in a different questionnaire. They can be as simple or complex as you make them, and direct users to any node on other questionnaires.
- Select theQuestionnaire that contains the target node.
- Select the Node within that questionnaire.
- Create Rules that trigger this navigation to occur. For example, if you want to navigate to a node based on the Case City field on the intake: select Case City, equals, and add the cities. If the Case City field on the intake is in any of these cities, then this navigation will occur. See this article for complete information about navigation rules.
- Click the plus button to add more navigation rules as necessary. Note: Click the plus sign and add multiple values with the "or" operator depending on the operator and answer type of the selected field.

- Save the questionnaire.
Example of a navigation rule:
You can create as many navigation rules as necessary. They can reference any question in the database and any field on an intake or matter record. Here's a questionnaire node with multiple navigation rules that allows the current node to branch into many different questionnaires.

Repeat steps 5 and 6 until you have all of the questions and nodes you need in your questionnaire.
